The applicants shall be required to produce following documents in Original alongwith one set of self-attested photocopies at the time of admission:

• Rank card (Only for B. Pharm, Lateral entry and M. Pharm)
• 10th Standard Marks and Certificate (Self-Attested Photocopy)
• 12th Standard Marksheet and Certificate (Self-Attested Photocopy)
• Pharm marksheet and certificate for M. Pharm (Self-Attested Photocopy)
• Pharm and certificate for B. Pharm Lateral entry(Self-Attested Photocopy)
• School/college Leaving Certificate (Original)
• Conduct certificate (Original)
• Migration Certificate (if applicable) 

Identity Proof:

• Aadhaar Card (Self-Attested Photocopy)
• Passport Size colour Photographs – 05 nos
• Stamp size (black and white) photographs – 02 nos

Other Relevant Documents:

• Caste Certificate (if applicable) issued by the competent authority(Self-Attested Photocopy)
• Domicile Certificate (Self-Attested Photocopy)
• Income Certificate (if applicable for scholarships) (Self-Attested Photocopy)
• Non-Creamy Layer Certificate (OBC) (if applicable)
• For lateral entry and M. Pharm students, state council registration certificate 
• Form of undertaking to curb menace of ragging
